Findings are not the deliverable — a recommendation is
Gathering facts (from a ticket, web research, or the codebase) is necessary but not sufficient. Every discovery run must convert what was found into an actual, actionable recommendation: a go/no-go (or build/don’t-build/build-smaller) stance, a concrete proposed direction, and your best-supported answer to each open question — not just a list of open questions with no answer attempted. See output_rules.md’s “recommendations.md is mandatory” section for the required structure. A run that only hands back raw research with no proposal or answer is an incomplete discovery pass, regardless of how much investigation went into it.
Challenge the idea before endorsing it
Before writing a positive recommendation, actively argue against it: does this duplicate something that already exists (a tool, vendor, or internal feature)? Is the problem actually painful enough to justify the effort? What would a skeptical stakeholder say? A recommendation that never seriously considered “we don’t need this” is not trustworthy — see output_rules.md’s required ordering for recommendations.md.
Problem → outcome → solution
Keep these layers separate in every artefact you write:
- Feature request (input only) — what was literally asked for; not yet validated as the right thing to build
- Symptom — the visible complaint or friction (may not be the real problem)
- Problem — whose pain / job; why it matters now; the underlying cause, not just the symptom
- Root cause — why the problem happens at all (at least a provisional hypothesis, cited to evidence)
- Consequence — what actually goes wrong when the problem occurs (delay, cost, error, escalation, churn, etc.)
- Outcome — measurable or observable change if this succeeds
- Opportunity — the intervention area the evidence actually supports
- Solution ideas — options under consideration (see
decision_and_governance.md’s solution-classes catalog — don’t assume “build something new” is the only option) - Committed solution (for build) — only what sources/decisions actually support
Do not leap from a stakeholder feature request straight to “the solution” without working through problem → root cause → outcome first. A feature request is an input to discovery, not a pre-validated requirement.
Four big risks (plus domain-conditional risks when they apply)
Use these consistently in the discovery plan, open questions, experiment design, and readiness sections:

Add these two only when the domain genuinely warrants them (e.g. safety-critical, regulated, or compliance-sensitive processes — do not add them by default for a low-stakes internal tool):

For each material unknown: name the risk type, its severity (High/Med/Low/Unknown), and the lightest adequate evidence bar (e.g. stakeholder confirmation, workflow observation, prototype feedback, technical spike, compliance confirmation, metrics baseline). Do not demand the same rigor for every TBD — high consequence needs stronger evidence, low risk needs lighter confirmation. See evidence_and_methods.md for evidence classes and confidence levels to label findings with, and decision_and_governance.md for matching an experiment method to each risk type.
Iteration protocol (when outputs/discovery/ is already seeded with prior published content)
Before writing anything, check outputs/discovery/ — on an iteration run it already contains the last published Confluence content (seeded by the pre-CLI step), not an empty folder. When it is non-empty, produce a delta and edit those same files in place instead of rewriting from scratch:

Update the relevant outputs/discovery/*.md file(s) in place with the merged result (prior content + delta) — since these files already contain the prior published state, this is a direct edit, not a copy-then-merge step. Note the delta itself in outputs/discovery/index.md’s “Last iteration” section too — don’t leave the delta only in chat/log output, since Confluence readers only see the published files.
Append-only artifacts are the one exception to “edit in place”
Most files above get merged/edited in place. Two optional files work differently — they exist specifically to preserve history across many iterations, so a run must never rewrite or drop their prior rows:
decisions-log.md— running Q&A/decisions record. Append new rows for what this run surfaced; never edit or delete an existing row (if an old open question now has an answer, add a new row referencing it).references.md— consolidated external-artifact index. Append new rows for new artifacts; never remove an old row even if that artifact’s topic later drops out of scope (mark it accordingly instead).
See output_rules.md for the exact table shape and when to bother creating these files at all (skip them for a single-session, one-shot discovery with no back-and-forth).
Working rules
- Source of truth: ticket description/comments/attachments for “what was asked”; any content already seeded in
outputs/discovery/for “what was already published”; any Confluence/URL the ticket or its comments point to for “what is documented elsewhere”. Call out conflicts across sources — do not silently pick a side. - No invention, not no investigation (see
investigate_before_writing.md): only state what sources support — but go actively find those sources first (web research for named external topics/technologies, CodeGraph for codebase questions, fetching linked docs). Use TBD or Open question only for what’s genuinely unknowable after investigating, not as a shortcut for skipping research. - Preserve detail: do not drop field values, status rules, interface names, owners, exception paths, or decisions. Prefer tables for contracts and mappings.
- AS IS vs TO BE: keep current-state and future-state clearly separated; label working assumptions.
- Traceability: every material finding cites its source (ticket comment, attachment name, Confluence section, prior context-pack entry).
- Experiment over documentation: when the dominant risk is Value or Usability, prefer proposing the cheapest test (interview, walkthrough, prototype, spike) over writing more pages.
- Trio check: before marking discovery “ready for delivery”, note whether Product / Engineering / Design-UX (or the closest available roles) have reviewed the dominant risks — record who reviewed, or mark pending. Do not invent reviewers.

What “investigate” means per mode
- Opportunity assessment / domain brief / source distill: if the ticket references a named technology, product, tool, competitor, or external concept (e.g. “analogs of X”, “integrate with Y”, “similar to Z”), you MUST look it up — use available web-fetch/browsing tool access to find real, current information about it (what it is, alternatives, how others use it, pricing/licensing if relevant, technical constraints). Summarize what you found with a citation (URL), not “TBD — could not research.”
- Discovery plan / AS IS-TO BE / mapping / PRD: if the ticket references an existing system, integration, or codebase area, use CodeGraph (
codegraph context "<topic>",codegraph query,codegraph callers,codegraph impact) to find and read the actual relevant source before describing current behavior — do not guess or leave it TBD if the codebase already has the answer. - Any mode: if the ticket or its comments link to a Confluence page, external doc, or spec, fetch it and read it — do not just cite its existence.
Codebase investigation — six required distinctions
Before proposing anything as “new,” resolve these six questions against the actual codebase/configuration — don’t skip straight to “we need to build this”:

A capability can be implemented but not enabled, or enabled but not used, or used but not known about by the people who’d benefit — these are different findings with different fixes (configuration change vs. training/discoverability vs. genuinely missing functionality), and conflating them leads straight to recommending a costly build for what’s actually a config or awareness gap. This directly feeds the “duplicate risk” check in recommendations.md’s challenge section.
No invention vs. no investigation — these are different rules
“No invention” (see general_guidelines.md) means: do not fabricate business decisions, user personas, metrics, or requirements that no source actually states. It does not mean “do the least possible research.” A thin 1-2 line ticket is exactly the case where investigation matters most — the job is to go find out what’s actually knowable (from the web, the codebase, linked docs) and only mark something TBD once you’ve genuinely looked and it’s still unknown.
Self-check before writing outputs/discovery/*.md
Before writing any file, ask: “If I only read the ticket’s own words back, would a reviewer say I actually did discovery?” If the honest answer is no, go investigate more (web search/fetch, codegraph, linked docs) before writing.

Why this matters
“I found evidence” means very different things depending on where it came from. Labeling every finding with its evidence class prevents the single most common discovery failure: treating a hunch, a vendor claim, or one person’s opinion as if it were confirmed fact. When writing a finding, name its class inline or in a table column — e.g. “(observed)”, “(reported — 1 stakeholder)”, “(implemented, not yet confirmed used)”.
Confidence guidance
Confidence is not a vote count — weigh source quality, independence, directness, and scope:

Turning assumptions into neutral research questions
Before designing an interview, survey, or research task, convert every assumption into a neutral question that could disconfirm it — never phrase it to confirm a preferred answer.

Research firewall — don’t let the pitch become the finding
Whoever runs the research (a human interviewer, an external research task, a deep-web-search prompt) must receive a neutral assignment, not a request to validate a preferred solution:
- Good assignment: “Identify current approaches to [the underlying problem], including evidence for AND against automation/a new tool.”
- Bad assignment: “Find evidence that [our proposed solution] is valuable.”
Context can shape which questions get asked; it must never become the answer itself.
Method selection — matching evidence need to method
Different questions need different evidence-gathering methods. Don’t default to “just ask people” for everything:

Scenario-based questions beat hypothetical ones
Prefer asking someone to show or reconstruct a real recent case over asking what they’d hypothetically do:
- Prefer: “Show me the most recent case where [the process] broke down or needed an exception.”
- Avoid: “Would you use an automated feature for this?”
People are unreliable predictors of their own future behavior with a not-yet-built thing; they’re good at recounting what actually just happened.
Atomic, falsifiable evidence — not vibes
Each evidence statement should support exactly one defensible, falsifiable finding — not a vague conclusion:
- Bad: “The process is inefficient and needs automation.”
- Good: “The user opened three separate tools (the primary system, a spreadsheet, and a chat app) before completing the task — observed in 2 of 3 sessions.”
Searching for evidence against
For every material problem or opportunity, actively look for counter-evidence before writing a conclusion: contexts where it does NOT occur, a successful current workaround, low measured frequency, an existing capability that already covers it, or a contradictory account from another source. Do not suppress a contradiction to make the write-up cleaner — surface it (see general_guidelines.md’s “No silent contradiction resolution”).
Cross-segment / cross-customer comparison
When a discovery spans multiple customers, sites, teams, or segments, build an explicit comparison table rather than blending everyone’s input into one narrative:

Solution classes — don’t default to “build something new” or “add AI”
When proposing a direction in recommendations.md or prd.md, generate options across these classes and explain why the cheaper ones were ruled out, not just why the chosen one works. Do not assume new software, and do not assume AI, is the preferred mechanism merely because it’s available or novel — see the BEFORE checklist above, roughly cheapest-to-most-expensive.
Automation boundary — classify any proposed automation
Whenever a solution involves automating a decision or action, classify it explicitly — do not casually call something “AI-powered” without first checking whether it needs to be:

Test first whether the actual problem is missing structure, missing data, fragmented rules, poor interface design, or weak integration — before concluding “this needs AI.”
Brainstorming techniques (pick what fits, don’t force all of them)

Experiment design — match method to the risk being tested
Don’t default to “user interview” for every risk — pick the method that actually reduces the dominant risk:

For any higher-stakes automation, prefer the safer, reversible methods first: historical replay → shadow mode → human-approved pilot → parallel run with explicit error analysis and rollback — before a full, irreversible rollout.
Measurement contract — define these for every experiment, before running it
Baseline · Target · Success metric · Guardrail metric (what must NOT get worse) · Safety metric (if applicable) · Adoption metric (if relevant) · Measurement source · Known data-quality limitation.
Classify the result as exactly one of: Passed / Failed / Inconclusive / Invalid experiment (the test itself was flawed) — never quietly reinterpret a failed or inconclusive result as a pass.
Readiness — use a 4-state verdict, not a binary one
A binary “ready/not ready” hides the realistic middle ground. Use:
- Not ready — material unknowns remain with no plan to resolve them
- Conditionally ready — ready to proceed IF specific named conditions are met first (list them)
- Ready with accepted residual risk — proceeding, with a named, explicitly accepted risk and owner (not silently ignored)
- Ready — dominant risks addressed to the team’s satisfaction, no material residual risk
A long PRD or a full set of discovery pages is not, by itself, evidence of readiness — judge against the actual risk evidence, not the page count.
Publication / sensitivity governance gate
Discovery input (ticket comments, interview notes, attached documents, code) can contain information that should not go into a broadly-readable Confluence page as-is. Before publishing, check for and handle:
- Personally identifiable information (names, contact details, or other individually-identifying data about a real customer/participant/employee) — replace with a role or a research ID (e.g. “Participant 3”, “Customer B”) rather than a real name, unless the ticket/source explicitly says publishing the real identity is fine.
- Credentials, API keys, tokens, internal endpoints, or other secrets — never copy these into a discovery page even if they appeared in the source material; redact and note that they were redacted.
- Direct quotations from a real person — only include if there’s a clear indication consent was given for that quote to be shared/published; otherwise paraphrase without attribution.
- Cross-customer/cross-segment comparisons that could reveal one customer’s specifics to another audience — anonymize (e.g. “Customer A” / “Customer B”) unless the intended audience is authorized to see real names.
- Any domain-sensitive conclusion (legal, safety, compliance, medical, financial) — flag it as needing human review rather than stating it as settled fact.

Use this table shape, inline within whichever file the decision belongs to (discovery-plan.md, prd.md, or a dedicated decisions/<topic>.md sub-page if there are several such decisions worth their own pages):

Do not invent additional top-level files beyond this table — but DO create subfolders under any of these when a topic’s complexity genuinely warrants splitting it into several detail pages (e.g. as-is-to-be/<stage-name>.md per stage of a multi-stage process, or mapping/<system-pair-name>.md per pair of integrated systems). decisions-log.md — optional, append-only running log
Some discoveries are not a single AI session but an ongoing thread across many real conversations/meetings over weeks or months (the ticket accumulates comments, stakeholders answer questions in follow-up rounds, new decisions get made). For this kind of discovery, maintain outputs/discovery/decisions-log.md as a table:

When a discovery accumulates several external artifacts (design files, recorded calls, spec documents, example data files, diagrams, third-party docs), maintain outputs/discovery/references.md as a single table so a reader doesn’t have to hunt through every sub-page to find the source material:

Most discoveries don’t need this file. Create it only when the source material (ticket comments, attachments, interview notes) contained something covered by decision_and_governance.md’s publication/sensitivity governance gate — e.g. a real person’s name/contact info, a secret, an unconsented direct quote, a cross-customer comparison, or a domain-sensitive (legal/safety/compliance) conclusion. Record: what was found, what was redacted/anonymized/paraphrased, and anything that couldn’t be resolved this run and still needs a human decision before broad publication.
recommendations.md is mandatory
Write this file every single run, no matter which mode(s) you ran or how thin the ticket was. A discovery pass that only hands back gathered facts/pages with no actual answer is incomplete — the deliverable is a recommendation, not raw research.
Required content, in this order (the challenge step MUST come before the bottom line is finalized — see below):
- Challenge the idea first — before concluding anything, actively argue the OPPOSING case: why this should NOT be built / why “no-go” or “kill it” could be right. Required sub-questions:
- Does this duplicate an existing feature, tool, or vendor already in use (internally or in the market) that solves the same job cheaper/faster?
- What is the cost of doing nothing, and is it actually worse than the cost/risk of building this?
- What’s the strongest steelman argument a skeptical stakeholder would make against proceeding?
- Only after writing this section does the bottom line get decided — if the challenge case is stronger than the case for proceeding, the bottom line MUST reflect that (no-go / kill / defer), not a “go” reached by ignoring your own challenge.
- Bottom line (one sentence): go / explore-further / no-go, or — for a feature/task-shaped ticket — build it / don’t build it / build a smaller version, stated as a direct answer, not a hedge, and consistent with the challenge section above.
- Proposal — the specific direction being recommended: which feature/approach/technology/next step, in concrete terms a stakeholder could act on today (not “we should look into options”). Use a numbered list or table for a multi-step proposal (phase | timeline | outcome) — Mermaid is NOT the default here (see
formatting_rules.md’s Mermaid caveat). - Why — the 2-4 findings (from your investigation — web research, codebase, linked docs) that most drove the recommendation, each with a real Markdown link to its source.
- Evidence against — the material counter-evidence you found (see
evidence_and_methods.md’s “Searching for evidence against”) even if it didn’t change the bottom line — do not suppress a contradiction just to make the recommendation read cleaner. - Answers to open questions — for every open question you would otherwise raise, give your best-supported answer/hypothesis first (labeled
**Recommended answer:**, with rationale and confidence), and only fall back to a bare unresolved “Open question — needs human input” for the genuinely undecidable ones (e.g. depends on a business decision, budget, or legal/compliance call no source can answer). A table (question | recommended answer | confidence | source) is preferred over prose. - What could change the recommendation — the specific new evidence that would flip it (keeps the recommendation honest and falsifiable, not just an opinion).
This is not the same as “no invention” (see general_guidelines.md) — a recommendation is explicitly your reasoned judgment call, clearly labeled as such, built on the facts you gathered. Labeling it as a recommendation (not a fact) is what keeps it honest. The challenge step exists specifically to counter confirmation bias — do not skip it or treat it as a formality to get through before writing the “real” (positive) recommendation.
Cross-linking
Link between your own files with plain relative Markdown links, e.g. [see the PRD](prd.md) or [mapping details](mapping.md) — the Confluence sync step rewrites these into real Confluence page links automatically. Do not use absolute file paths or guess Confluence page IDs yourself.
The link text must always be a natural-language phrase — never the bare filename. This is not just a style preference: Confluence’s link-safety UI shows a “Check this link — this is taking you to a different site” interstitial click-through warning whenever the visible link text doesn’t resemble the resolved destination, and a bare filename (as-is-to-be.md, prd.md) never resembles a Confluence page title/URL, so it reliably triggers that warning on every single click — even though the link itself is a correct, internal, same-space reference. A natural-language phrase describing what the reader will find avoids this entirely.

If you want the raw filename visible too for orientation, put it in a separate non-linked `filename.md` note next to the real link, don’t make the filename itself the clickable text.
Re-runs / iteration
On an iteration run, outputs/discovery/ is NOT empty when you start — the pre-CLI step seeds it with the last published Confluence content, recursively at every depth of the existing page tree (same file/subfolder naming as the table above), before you run. Read what’s already there first, including any nested subfolders. Then edit the same file in place with the merged result (old content + this run’s delta) rather than deleting it and starting fresh — this keeps the same Confluence page updated instead of creating a duplicate tree, and guarantees nothing you didn’t intend to touch gets silently dropped. This applies to recommendations.md too: re-affirm, sharpen, or revise the recommendation based on any new artefact — don’t leave a stale recommendation unexamined.
Exception — append-only files: decisions-log.md and references.md are never rewritten wholesale on iteration, even though they’re seeded the same way as everything else. Read their existing rows, then only append new rows for what this run actually added — see their own sections above for why (they’re a historical record, not current-state).

Rich formatting is required, not optional
A wall of plain, unformatted paragraphs is a formatting failure, even if the content itself is accurate. Every file should look like a real Confluence page, not a text dump:

Tables over prose for structured data
Prefer a table whenever you are recording: risks (risk type | severity | evidence bar | status), decisions (decision | date/source | status), open questions (question | risk type | evidence bar | status), competitor/alternative comparisons (option | strengths | weaknesses | pricing/notes), or field/interface mappings (field | source | target | notes).
Mermaid diagrams — NOT the default, only as a carefully-fenced fallback
Confluence Cloud does not natively render Mermaid diagrams — it requires a marketplace plugin that most spaces don’t have installed, and this agent has no way to know whether a given project’s Confluence does. Writing raw Mermaid syntax expecting it to render as a diagram produces exactly the opposite of “richer formatting”: either broken/garbled plain text (if not code-fenced) or, at best, an unrendered monospace code block showing diagram syntax nobody asked to read.
Default to tables and numbered/bulleted lists for flows, timelines, decision sequences, and comparisons — these render correctly on every Confluence instance with zero risk. Reserve Mermaid for the rare case where a table genuinely cannot express the relationship (e.g. a branching decision tree), and even then:
- Always wrap it in a proper fenced code block:
```mermaid…```— never as bare unfenced text (which is what produces garbled prose, not even a readable code block). - Immediately follow it with the same information as a table or list, so the content is still fully readable even if the diagram itself doesn’t render on the target Confluence.
Links and references are mandatory, not decorative
Every finding sourced from the web, a linked Confluence page, or a codebase file must be a real Markdown link, not plain text naming the source:
- Web research:
[Vendor X product docs](https://example.com/docs), not “source: Vendor X docs” or a bare pasted URL with no link syntax. - Codebase findings:
[UserService.java](../src/main/java/.../UserService.java)style reference (or the file path in backtick-code if it isn’t a resolvable relative link from the output folder) plus the relevant symbol/line if known. - External non-web artifacts (a recorded call, a shared spec document, a design file, an example data file, a diagramming-tool link) — link them the same way:

Evidence and decisions must be traceable
Every row in a decisions/assumptions/open-questions table needs a source column with a real link where one exists (a URL, a Confluence page, a file path) — “ticket comment 2026-07-20” is acceptable only when there is genuinely no linkable artifact (e.g. a verbal/chat comment with no URL). Never leave the source column blank.
Findings must be atomic and falsifiable, not vague vibes
A finding should support exactly one defensible, checkable claim — not a sweeping conclusion:
- Bad: “The process is inefficient and needs automation.”
- Good: “The user opened three separate tools (the primary system, a spreadsheet, and a chat app) before completing the task — observed in 2 of 3 sessions.”

Self-check before finishing EACH file (not just recommendations.md)
Every single file in outputs/discovery/ — not only recommendations.md — needs this check. See modes.md’s “Per-mode required structure” for the specific table(s) each mode’s file needs. Before moving to the next file, verify:
